Request A Prescription Refill for Your Pet

Thank you for filling out this form to request a refill of your pet’s prescription.

Important Notices

Your refill request is not confirmed until one of our client care team members has either contacted you via phone, email or text to inform you the medication is ready for pick up; or to follow-up with more questions as requested by the veterinarian.

We ask that you provide a minimum of 48 hours for all prescription refills requests. There are some medications that we require more than a weeks notice for refill because they need to be compounded at a separate pharmacy. If you are unsure how long your pet’s medication takes to be prepared please contact the clinic and inquire with our client care team.

We need to have an on-going doctor/patient relationship through annual examinations in order for our veterinarians to prescribe any medications. If your pet requires lifelong medication please ensure with our client care team that your pet’s annual examination is up to date.

Some medications require regular diagnostics and examinations (i.e. blood work, urinalysis, eyes/ears exams) in order to ensure your pet is healthy enough to handle potential side effects of some prescriptions and the medication is appropriate for your pet’s current condition.
